Anthem, now operating as Elevance Health, has reached a $12.875 million settlement in a class action lawsuit. The lawsuit alleged that Anthem improperly denied coverage for residential treatment services for mental health and substance use disorders by using overly restrictive criteria. While Anthem denies any wrongdoing, they agreed to the settlement to avoid further litigation. The final approval hearing is scheduled for January 26, 2026.

Individuals who had claims for residential treatment denied between April 29, 2017, and April 30, 2025, may be eligible to claim funds. Eligible customers can receive a pro rata share of the Out-of-Pocket Reimbursement Fund or a nominal payment of at least $100. To claim, individuals must provide proof of out-of-pocket payments made within 14 days of a qualifying denial and submit a claim form by January 20, 2026.

Class members who do not opt out will automatically receive a nominal payment but will forfeit their right to independently sue Anthem regarding the issues addressed in the lawsuit. More information and the claim form are available on the settlement website.
